Family ID Registration

Families can register students online for athletic team participation through the Family ID website.

This secure registration website allows families to register and upload required medical forms. Families should register for each sport season when registration opens.

Physical Exams

Students must have an up-to-date physical on file in the Schalmont Health Office (physicals are valid for one year). A private physical with a student’s family physician is recommended as they are most familiar with the student’s medical history.

A Health Examination Form is available here. Please take it to your family physician. It must be reviewed by the school nurse before a student can practice.

Cardiac Arrest Prevention

Schools are required by New York State to ensure that student-athletes and families are provided with information about sudden cardiac arrest. While occurrences are rare, there are steps that families and school personnel can take to identify students who may be at risk. Read more about Cardiac Arrest Prevention.
