sports clerance dates

Isabella Borini at statesStudent-athletes must fulfill a variety of requirements in order to participate in interscholastic sports.

Mandatory
Sports Clearance

Students are required to go to the health office on a specific date/time with their completed paperwork for sports clearance. At that time, the school nurse will review the student's latest physical date or their interval health history form. If the student is approved, a sports  participation approval form will be given to the coach before the first day of practice.

All modified, junior varsity, and varsity athletes must be cleared by the nurse in their school building before they may begin practice.

Spring Sports Clearance Dates:

The following mandatory sessions for junior varsity and varsity teams will be held in the high school health office.

  • Monday, Feb. 13 at 2:15 p.m. for Boys Track, Baseball and Boys Tennis
  • Tuesday, Feb. 14 at 2:15 p.m. for Girls Track, Girls Softball
  • Monday, Feb. 27 at 2:15 p.m. for Make-up date for all Spring Sports

Students must provide the nurse with their completed paperwork at the designated sports clearance date above. Paperwork Information

A sports participation card will be filed for all students approved by the school nurse. A copy will also be provided to the coach.

Sports physicals: 

A physical from a student's family doctor is highly recommended.
